최소 절개 함몰유두 교정 수술 학회 발표

2008년 5월 8일부터 10일까지 제주도에서 열린 대한성형외과학회에서 함몰유두에 대한 강의를 맡게 되었습니다.

금번 연자발표는 국내 성형외과 전문의들을 상대로 한 발표로 "실루엣 성형외과 윤상엽원장"이 연자로 나서게 되었습니다.

함몰유두 수술은 비교적 흔하며 많은 여성들이 고민하고 있는 부분으로 성형외과 의사 사이에서도 많은 관심을 받고 있는 가슴성형의 한 분야입니다.

실루엣 성형외과는 유륜에 흉터를 남기지 않는 최소절개로 많은 수술을 하고 있으며 수술방법에 대한 노하우를 학회 차원에서 성형외과 의사들을 상대로 발표하게 되었습니다.

The Correction of Inverted Nipple Using One-Hole Minimal Scar Method

Inverted nipples have been repaired by many surgeons using many different approaches.

There is a wide range of inverted nipple conditions.

Etiologies of the inverted nipple, congenital and acquired, and the pathophysiology have been described by many other surgeons.

It is characterized histologically by less fibrous connective tissue beneath the nipple and shortened and fewer functional lactiferous ducts.

In mild to moderated cases, it is my opinion that the major pathophysiologic basis of inverted nipples is shortened breast ducts and not lack of tissue at the base of the nipple.

Under satisfactory local or intravenous anesthesia, the nipple is everted and maintained in gentle traction using skin hooks.

A small incision is made on lateral side at the nipple-areola junction.

Inward and upward compression of tissue immediately beneath the nipple demarcates the level at which intervening ducts and connective tissues are then divided by sharp dissection only vertical direction.

Blood and nerve supply to the nipple are preserved superiorly and inferiorly in the bipedicle skin-dermal flap.

Two purse-string sutures are performed for the maintenance of corrected nipple.

The small incision is closed by the tissue adhesive.

Advantages to using this technique include

(1) no scars on the flat surface of the areola,

(2) adequate blood and nerve supply to the nipple by means of superior and inferior nipple-areola bridges,

(3) simple and easy to learning,

(4) no need of external stent or traction.
